COVID-19 Vaccines: Eligible CHOP patients ages 12 and up can schedule a COVID-19 vaccination appointment at their Primary Care office. To make an appointment: Log in to your MyCHOP account and choose the eligible child’s profile. Click “Menu” > “Schedule an Appointment” > “Care Network (Primary Care).” When selecting reason for visit, choose “COVID Vaccine Appointment.”
