Advanced Pediatric Surgical Techniques in Plastic and General Surgery

Overview

In our first lecture, Dr. Taylor will present evidence of how CAD/CAM modeling and planning is revolutionizing the practice of cranio-maxillo-facial surgery. Virtual surgical planning is changing both what we can offer patients as well as the level of accuracy with which we can carry out complex bony movements of the head and neck.

In our second lecture, Dr. Mattei will present on surgical options for children with ulcerative colitis or familial polyposis. Ulcerative colitis (UC) is an inflammatory disease of the colon. While most children are treated with medicine and do well, a small percentage will fail medical management and are offered surgery, which is curative but requires removal of the entire colon and rectum. Most are able to have a J-pouch reconstruction, which allows them to avoid a permanent ileostomy and essentially live a normal life, but this is a complex operation with significant risks. Children with familial polyposis have an inherited tendency to make polyps that puts them at high risk for colon cancer at an early age. Colectomy and J-pouch reconstruction is offered to many of these children as well. We will discuss the CHOP experience with this operation, how it has evolved over time, and how our outcomes have improved.

Objectives

  1. Understand the indications for surgical intervention in children and adolescents with UC or FAP
  2. Appreciate the short- and long-term risks and benefits of total colectomy and J-pouch reconstruction
  3. Understand the indications, kaleidoscope of utility, and potential pitfalls of CAD/CAM planning in cranio-maxillo-facial surgical planning and execution
